Appointment Information:
We welcome you to our dental practice and want you to know we value your business.  In an effort to provide the highest care in dentistry we require 48 hours notification for any scheduling changes that may arise.  We understand that emergencies do arise and we will do our best to accommodate any issue.  However, we want you to understand that missed or broken appointments are hurtful in many ways.  Missed appointments delay your treatment and dental care needs and our ability to keep your oral health at optimum levels.  They also may prevent us from seeing another patient who needs treatment from getting the necessary care in a timely manner.

A Message about Dental Benefits:
Dental Insurance is meant to be a partial reimbursement for basic care(basic preventative and basic restorative.) Most adults and many children need more than basic dentistry.  Fees are often determined by the plan chosen by the employer or employee.  While we take most insurance plans, it is your responsibility to understand your coverage.  We are willing to assist you in maximizing your benefits to the best of our abilities.  Please understand that we do not allow your insurance to determine how we provide your treatment.

Our jobs as healthcare practitioners, is to provide the best comprehensive care to you as a patient.  We are happy to file your claims to your benefit plan, however you are responsible for keeping our staff informed of any changes that occur in your plan.  If there is a problem with delayed payments for settlement, you will be billed for the outstanding balance of services rendered.


What To Expect
On the day of your first visit to our office, it's always a good idea to arrive a few minutes early. This allows extra time for finishing up any registration paperwork and ensures you have plenty of time to familiarize yourself with our office, meet your doctor and get started with treatment. To save time, download and complete the new patient forms above and bring them with you to your first appointment. During your initial visit we will discuss your needs and goals to help us determine a personal treatment plan for optimal health and wellness.
